Job Summary:
Legacy Healthcare LLC is seeking a compassionate and skilled Medical Social Worker to join our Hospice team in Alameda County. As an MSW, you will provide high-quality care and support to patients with life-limiting illnesses in their homes, assisting them to maintain their comfort, dignity, and quality of life. Our team is dedicated to providing exceptional care and support to our patients and their families, and we are looking for a dedicated and compassionate individual to join our team.

Responsibilities:
- Directs patient social service operations including providing direct oversight of the establishment and implementation of social service policies.
- Serves as liaison between patients/families/caregivers and community agencies.
- Assures proper documentation of social services in clinical records
- Maintains comprehensive working knowledge of the field of social work
- Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources
- Communicate with other members of the interdisciplinary team to ensure all patient and family care needs are met.
- Work in collaboration with the Interdisciplinary Team in the planning, implementation, and evaluation of medical and nursing care.
Requirements:
- A Master's degree in Social Work (MSW) of an accredited program
- Current California Associate Clinical Social Worker license
- Valid driver’s license, proof of acceptable automobile insurance coverage, and reliable transportation (mileage reimbursement provided)
- Previous experience in hospice preferred
- Ability to work independently and as part of a team
- Excellent communication and interpersonal skills
- Ability to lift up to 25 pounds and assist with transfers and ambulation as needed
- Reliable mobile phone with a plan that provides good local coverage.
- Basic computer skills
